Something I saw in draft the other night, and I'm not quite sure how it should technically be addressed:

Player controls a Champion's Drake, or any other generic beater.
Opponent controls a Guard Gomazoa, or any other blocker.
Player plays a Narcolepsy on the said blocker, and both players mistakenly let it be tapped, forgetting that it doesn't tap right then. So, under the assumption the opponent has no blocker, Player swings, but then for blockers somebody realizes that the gomazoa shouldn't be tapped.

Not that this should require a judge in-game, I would just be under the assumption that the gomazoa becomes untapped, as it should have been in the first place, but that the player should get to reconsider the attack. I can't see it making sense that the defending player would be allowed to fix the error and then block, but it was an interesting scenario that I was curious about.

There are a few different general situations, each of which have their own procedure. Read the IPG (http://www.wizards.com/default.asp?x=dci/doccenter/home) for full details.

For Game Rule Violations only, if the game state can be backed up to the point of the violation, the Head Judge may choose to back up the game state. Only the Head Judge can make this decision. The game state is backed up completely or not at all - no partial "fixes". If a lot of decisions have been made based on the incorrect game state, cards changing zones, etc, it can't be backed up.

In your example, the game would be backed up to the point where the Gormazoa was mistakenly tapped.

If a trigger is missed that's non-optional, has an effect on the visible representation of the game, and has a default choice, it's resolved immediately without using the stack using the default choice. If there's no default and it's still within a turn cycle, the trigger is put on the stack immediately.
