Creatures and Counters

If have have a creature with +1/+1 counters on it, such as a Vinelasher Kudzu, when it goes to the graveyard, is it's Power/Toughness considered the total of it's printed plus the counters it has gained?

As it goes to the graveyard, its P/T is what it was when it was last in play, with all the counters. When it's officially found it's home in the yard, Vinelasher would be a 1/1.

ex. You have a Vinelasher Kudzu with 6 +1/+1 counters on it and you sacrifice it to Momentous Fall, you would draw 7 cards and gain 7 life. That same Vinelasher can then be returned to play with a Reveillark.
