I know this is already discussed with Smokestack but what about this scenario:
I have Enduring Ideal on epic mode. So at the beginning of my upkeep, a copy of it goes on stack. It resolves, then I put Form of the Dragon in the field. Does Form of the Dragon's trigger still trigger?

No, the turn that you put Form of the Dragon into play you don't get to do 5 damage. This is because it was not in play at the beginning of your upkeep to trigger. It will trigger at the beginning of future upkeeps though.
