when you cast surgical extraction do you target a card when you declare you are casting it. or do you say what you are naming after the spell resolves?

Yes, since Surgical says "Choose target card in a graveyard...".

Any time a card uses the word "target" you have to choose the target when it's put on the stack, not upon resolution.
