Elvish Spirit Guide.

The casting cost and uniquely distinguishable card name are included for this card. Contagion only appears by inference and without a discrete mana cost.

Yes, good point. It's really going to be the HJ's decision. When in doubt - ask the HJ about questionable card alters/miscuts/sleeves.

The name on a card determines what card it is interpreted as. The example is an Elvish Spirit Guide because more of the name is visible than the other name.

3.6 Card Interpretation
The official text of any card is the Oracle text corresponding to the name of the card. ...

A head judge can deny the use of any misprinted or altered card if they feel it would cause confusion.
