Playing in a standard FNM with valakut, I think before titan even came out
My opponent is using a mono W control deck that plays eldrazi temple and such to ramp into emrakul.
My opponent is on about 10 or 11 life because I've been attacking him.
My only creature right now though is an ondu giant. I have 2 valakuts and a khalni heart expedition with 3 counters on it but I only have 3 mountains in play so cracking it won't do anything.
He casts emrakul, so I guess it's all over, but I don't concede

I don't often play standard, I think that's the only time I ever won an FNM (all the good players were playing in the cash tournament at the store up the road)

I'm sure I have made some pretty dumb errors too, everybody makes mistakes and I'm sad that I can't really remember any of them because being aware of your shortcomings is how to improve
I've had an opponent turn two a standstill on me when I was playing Lands. I think I even played Exploration turn 1. I looked at him and said "I'm pretty sure I'm going to be drawing cards off that." A few turns later I'm beating in with two Factories after wasting the two Mutavaults he thought would get me to crack the standstill. He plays some merfolk, I draw 3, and he asks to go to game two not long after.

This weekend my friends and I had a chaos draft. I went an unspectacular 2-2 with a UBw control deck, but my friend and organizer (let's call him D) 3-0'd with the sickest GU deck ever. In the final round, he was up against GWB fat stuff.
I walk over and it's turn six or so of game 2. D has a Momir Vig and a Deepwood Drummer in play. His opponent casts Spined Wurm. The following happens:
D: Time Ebbs Spined Wurm, attacks.
Opp: untaps and casts Spined Wurm.
D: Memory Lapses Spined Wurm, attacks on his turn.
Opp: untaps and casts Spined Wurm.
D: Whirlpool Whelms Spined Wurm EOT. The clash reveals Annex for D and something that costs less than four for the opponent.
D: untaps and Annexes opponent's Simic Growth Chamber, attacks.
It was brutal. Unfortunately, then D drew six lands in a row and lost to the Spined Wurm (which finally stuck) and an Alpha Tyrannax.
Me: "Land, go."
Them: "Land, Thoughtseize."
Me: "Dark Ritual, Dark Ritual, Ad Nauseam, stop at 2 life."
Them: "I'm not scooping, take Tendrils."

At the Dutch NC legacy, some weeks ago, I played a mono black suicide-type deck incl. smallpox and Death's Shadow (which is such an awesome card!).
I play BUG thresh in round three, standing 1-1 so there was still hope (yeah right...). Board state was about equal when I play smallpox and he sacrifices his only creature, a nimble mongoose while having threshold. I have a Snuff Out in hand and neither of us has any creatures. He then draws mongoose, casts it and says go. I draw irrelevant stuff and pass. He draws mongoose, casts it and attacks with the other one. I draw irrelevant stuff and pass. He draws mongoose AGAIN, casts it and attacks with the other two. Now my life total is at 9. I draw Death's shadow, which is now 4/4 so I will be able to survive one attack. He draws, attacks for 6 and I ask him: "You drew a lightning bolt, right?" after which he apologizes and reveals the bolt. P_)(^#%
I would have drawn Tombstalker, stabilizing the board at that moment. Stupid shrouded mongooses, that snuff out was totally useless!
The next match I made an interesting mistake. I'm paired against goblins, we are at 1-1 and have a very close third game. At one point I have a 5/5 Death's Shadow and a Dark confidant in play and he has 3 goblins. Since his turn would probably the last he draws and says "I just go for it" and swings with 3 of his goblins (Matron, Ringleader, 2/2 dork). I'm so afraid he drew the gempalm goblin so that, if I would block a goblin with my death's shadow, he would be able to deal lethal damage to it, removing my finisher. So I decided to take the 5 damage, going to 4. He then reveals Lightning bolt (totally forgot about that card, I was convinced he couldn't hurt my life total) and says YES I WIN! While I point out I still live and have 1 life left. At that stage I was like YES I WIN! when he pointed to me the Dark confidant still in play, hoping it would reveal a lethal card. I was just about to use Top when I realized I used it to draw a card, so confidant would kill me for sure. I reach out my hand, he shakes it, thinking he lost, when I pointed out Top was on top...
Now the mistake (who figured it out? :P): the Death's Shadow would never die from either Lightning bolt or Gempalm dude, since the other two goblins would hurt my life total, increasing it's P/T before he would die... Yeah, great call on that one!

Opponent: Shelldocks out Emrakul, pauses, looks at Ensnaring Bridge and scoops
Me: Reads Ensnaring Bridge more closely and realizes it affects all players

It's game 2 and he drops Leyline of the void. I freak inside my head as I realize I don't have a damned thing to get rid of that in the deck. I didn't expect him to board those since I saw no black game 1. I didn't board in Show and Tells since I don't want him dropping fatties larger than mine. Ie. Projo and Emrakul. I'm thinking I'm pretty locked out.
I decide to keep going to see if I can think of something.
He casts a cascade spell and I force the Hypergenesis to buy some time to think.
He casts another Hypergenesis off cascade next turn. I have to let it go.
He drops Progenitus. I drop Sheodred and smile a little. He is done. Wait?....done?
I get to drop Elesh Norn and Jin Gitaxias along with it.

IBA suggests Brightstone Ritual as a bizarre answer to Empty the Warrens, so I started testing it against some ETW tossing decks. Here's one of my best results ever and quite possibly the best turn one in the history of Vial Goblins:
